Social Worker Specified Purpose Contract (4 months) / 35 hours per we Location: Kilcullen, Co.
Kildare Salary Scale: Social Worker €50,120 to €70,980 DOE The Social Worker will work as part of a multidisciplinary team in the delivery of a Social Work service to people with an intellectual disability and their families.
Support and supervision will be provided to the person appointed.
Responsibilities: Conduct a comprehensive assessment of persons with a disability and their family.
Determine the most appropriate social work methods of practices that would best enhance the quality of life of the person with a disability To maintain accurate contemporaneous records of intervention, ensuring that they and all other documentation relating to services users is in accordance with professional and legal requirements To work as part of the Multi-disciplinary Team and engage in collaborative goal setting as a team.
Support individuals to enhance their life by ensuring goals and objectives outlined in the Individual Support Plan are achieved.
Individuals may have goals and objectives in a wide variety of life areas including but not limited to personal care, work and leisure, lifelong learning, owning a pet, visiting family, and going on holidays.
