Community Liaison Nurse required for Children's Disability Network Team based in Portlaoise.
Children's Disability Network Teams provide interdisciplinary team support to children with complex disability needs from birth to 18yrs.
They provide care through the Bio-psychosocial Model outlined in Progressing Disability Services and are part of the Midlands, Louth and Meath Social Care Division of Care in the HSE.
The team consists of a variety of health care professionals, administration support and the Children's Disability Network Manager.
The Liaison nurse post is part of the interdisciplinary team and is further developing new posts within new CDNTs, Some duties include but not limited too: • Manage client care to ensure the highest professional standards using an evidence based, care planning approach • Provide a high level of professional and clinical leadership and practise Provide safe, comprehensive nursing support to clients with complex disabilities according to the Code of Professional Conduct as laid down by the Bord Altranais agus Cnáimhseachais na hÉireann (Nursing Midwifery Board Ireland) and Professional Clinical Guidelines • Practice nursing according to: o Professional Clinical Guidelines o National and Area Health Service Executive (HSE) guidelines.
o Local policies, protocols and guidelines o Current legislation • Manage own caseload in accordance with the needs of the post • Participate in teams / meetings / committees as appropriate, communicating and working in co-operation with other team members • Facilitate co-ordination, co-operation and liaison across healthcare teams and programmes • Grade: CNM2 • Disciplines considered: RGN, RGN or RNID with relevant managerial experience essential to be considered • Service Hours : Monday to Friday • Times: 9-5 • Timeframe: 3 months with possible view to extension • Immediate start required • Full job spec attached To note your interest,
