The Role
This role involves providing care management services in an Irish home setting. The ideal candidate will have experience in social healthcare, preferably holding a QQI Level 5 Social, Healthcare degree.
The successful candidate will be responsible for:
* Care Planning: Conduct consultations, prepare care plans, and monitor progress to ensure effective delivery of care.
* Risk Assessment and Mitigation: Identify potential risks in the home environment and develop strategies to mitigate them.
* Client and Caregiver Support: Manage concerns, mediate issues, and provide guidance to both clients and caregivers.
* Quality Assurance: Conduct regular visits to ensure adherence to HCCI, ISO, Franchise Standards, local policies, and Health Service Executive requirements.
* Mentorship and Team Support: Guide a team of caregivers to enhance their performance and promote a collaborative work environment.
* Relationship Building: Develop and maintain strong relationships with clients, family members, and healthcare professionals.
* Reporting and Consultation: Share clinical insights with senior management and participate in organizational audits as required.
* Additional Duties: Assist with tasks delegated by the management team.
