Lead Nurse Coordinator Role
As a key professional in the rehabilitation service, you will play a pivotal role in coordinating patient discharge and ensuring seamless transitions of care between hospital and home.
* Conduct ongoing assessments of patients' clinical and social needs to determine timely readiness for discharge.
* Develop, implement, and monitor individualized discharge plans in collaboration with consultants, senior nurses, and multidisciplinary teams.
* Liaise with community nursing, primary care, social work, and allied health professionals to arrange post-discharge supports and services.
You will possess excellent interpersonal skills, be capable of communicating effectively and efficiently with a wide variety of departments and individuals, including patients and colleagues. You will also have the ability to deal with challenging situations and proactively influence outcomes.
