Job Description:
Social care professionals play a vital role in providing support to children and their families who require additional assistance. They form part of an interdisciplinary team, delivering high-quality services to those in need.
The primary responsibilities of social care workers include reviewing clinical reports from private providers, documenting outcomes and recommendations under the supervision of the Children's Disability Network Manager.
They also support transfer/referral to other services where appropriate, under the guidance of the Children's Disability Network Manager and clinical team.
Collaboration in the discharge of clients from the CDNT is another key aspect, with close working relationships between the Children's Disability Network Manager and clinical team.
Social care workers contribute to file reviews and preparation of clinical note summaries, under the supervision of the Children's Disability Network Manager and clinical team.
Administrative support and maintenance of the day-to-day operation of the service are also essential duties.
Additionally, they research and prepare resources for universal family support and strategies.
Participation in team meetings and reporting on matters affecting service delivery to the Children's Disability Network Manager is also required.
Maintenance of accurate documentation, adhering to local guidelines, principles of confidentiality, Freedom of Information (FOI), and GDPR Acts is crucial.
