Lead Care Coordinator
The main goal of this role is to provide person-centred care and support that meets the practical, physical and emotional needs of service users.
* Deliver supportive services to individuals with intellectual disabilities/ABI/Autism
* Assist persons with disability in residential, training, and day service activities
This position works collaboratively as part of a multidisciplinary team (MDT) for effective service delivery. The Team Lead will report directly to the Person In-Charge.
