Job Title: Intellectual Disability (ID) Nurse Location: Baldoyle, North Co Dublin Service: Adult Disability Service - Social Care Reporting To: Clinical Nurse Manager / Director of Nursing Job Purpose The Intellectual Disability (ID) Nurse will provide person-centred nursing care to adults with intellectual disabilities, promoting independence, dignity, and quality of life. The role involves supporting individuals with complex health, behavioural, and social needs within residential and/or community-based settings. Key Responsibilities Liaise with the Doctor and allied professionals in relation to health issues. To have good protocols in place for minor illness, prn use, long term conditions and the overall medical care of the residents. Follow up of medical appointments. Accompanying residents to GP, specialists or hospital appointments as needed and advocating for them around their health issues. Advising care assistants with appropriate questions to ask when accompanying residents to the doctor. Establishing with Care Assistants, an understanding of medical issues, and interventions conditions. Assisting Core Members in understanding their medical issues, their medication, and their health promotion. Assisting in Care Planning for Core Members. Monitoring progress in health issues. Arranging annual medical checks. Supporting Core Members who have mental health issues and referring them to the appropriate sources of expertise. Liaising with Mental Health PHN in relation to appropriate Core Members. Conduct risk assessments in relation to personal care and to treatments and long-term care. Assist in providing Person Centered Planning of Care and implementation of care plans Training in medical conditions and treatments associated with these conditions Training in Administration of medication, care of drugs and their side effects Assisting in the creation and updating of Policy, Protocols and procedures in relation to health issues Adhering to and training assistants in the policy relating to standard precautions checking blood glucose monitoring, blood and body fluid spills, needle stick injuries Providing advice (as needed) to house and workshop assistants Medical needs and health promotion needs. Supporting residents around mental health and referring them to the appropriate sources of expertise. G.P. Psychiatrist/Public Health Nurse for Mental Health/ Psychological Team Keeping up-to-date medical records for each Core Member. This will involve obtaining, reviewing and filing reports (e.g., following doctor's visits), ensuring resident's medical files are up to date and relevant people are informed of changes in health, and medication changes in association with house team. Liaising with the families of Core Members regarding health issues. In conjunction with the Key workers, house teams and where necessary the Person in Charge. Dealing with minor wounds/ illnesses, changing dressings for the core members. Health Promotion: Promoting health among the core members. This will include areas such as diet, exercise, hygiene, personal grooming, sexuality and relationships. Essential: Registered Nurse with NMBI (RNID) Experience working with adults with intellectual disabilities Strong understanding of person-centred care principles Excellent communication and interpersonal skills Experience in residential or community disability services Training in Positive Behaviour Support (PBS) Knowledge of HIQA standards and safeguarding procedures Full clean driving licence Skills: Compassionate ID Nurse Patient ID Nurse Empathetic ID Nurse Supportive ID Nurse Dedicated ID Nurse Respectful ID Nurse Observant ID Nurse
