Job Title:
A community support worker is required to empower service users to live independent lives in their home and the community.
* Key responsibilities will include supporting the development and delivery of person-centred plans, developing partnerships with local communities, and transporting service users to access community resources.
Requirements:
* A relevant qualification in healthcare or a related field (minimum Level 5).
* Experience working with individuals with disabilities or in a similar sector.
* Excellent command of spoken and written English.
* Full clean manual driving license with access to own vehicle for work purposes.
