Role Summary

This role plays a key part in informing placement decisions across Resilience Healthcare services by conducting comprehensive assessments and producing high-quality reports.

The successful candidate will report to the Business Manager Referrals and Contracts and be responsible for reviewing referral information, completing person-centred assessments, and producing clear and accurate assessment reports that guide decisions made by the Referral Committee.

Key Responsibilities:

* Review referral files to determine suitability for Resilience Healthcare Services
* Conduct comprehensive, person-centred assessments in community settings across Ireland
* Produce clear, accurate, and evidence-based assessment reports to support decision-making
* Recommend service models or supports aligned with Resilience care pathways
* Build rapport and trust with individuals and families during the assessment process
* Communicate professionally and empathetically with stakeholders

Requirements:

* 3+ years of experience in disability services, social care, behavioural support, clinical roles, or assessment-based work
* Recognised qualification in Social Care, Psychology, Applied Behaviour Analysis, Nursing, Occupational Therapy, or a related field
* Demonstrated experience conducting functional, clinical, or holistic needs assessments
* Strong report-writing skills and attention to detail
* Ability to work independently while being part of a supportive team
* Highly organised with strong time-management skills for on-the-road work
* Full clean driving licence and willingness to travel nationwide

Travel and Mobility: Travel throughout Ireland to complete assessments using a company vehicle provided for the role.

Delivering Results: Takes ownership of workload and strives for excellence, meets deadlines, and maintains high-quality assessment outputs.

Operational and Administrative Responsibilities: Maintain accurate, confidential records in line with GDPR and safeguarding obligations, update and manage assessment information on Dynamics 365 and other internal systems.

Service Knowledge and Continuous Learning: Develop a strong understanding of Resilience Healthcare's service models, conduct familiarisation visits to maintain up-to-date knowledge, and contribute insights to continuous improvement of assessment tools and processes.
