Job Description:
The Project Worker will support individuals through case management and key working, helping them meet their goals, address support needs, and sustain housing. Responsibilities include processing referrals, conducting assessments, care/support planning, engaging with service users, supporting move-on plans, managing maintenance issues, and participating in supervision.
What you will do:
1. Work according to the principles of Housing First and within the overall objectives of the Housing First Intensive Case Management Service.
2. Create a safe, welcoming environment for clients, respecting their choices and self-determination.
3. Build trusting relationships with clients, especially those living on the streets or in temporary accommodation, to facilitate their move to stability.
4. Conduct assessments and support planning using appropriate systems and tools.
5. Support clients with housing, welfare rights, and accessing move-on accommodation.
6. Assist tenants in creating and maintaining a home, supporting their adherence to tenancy indicators.
7. Maintain relationships with accommodation providers across social housing, homeless sector, and private rentals.
8. Support clients in appointments with other services and in developing life skills.
9. Manage caseloads, record casework activities, and input data into the Focus Ireland database.
10. Collaborate with team members and external agencies.
11. Attend supervision sessions and contribute to policy development.
12. Ensure health and safety protocols are followed and report concerns.
What you will need:
* Degree in social care, youth work, or related discipline.
* Experience working with homeless individuals and those with dual diagnoses.
* Team working experience, report writing, IT skills, and liaising with agencies.
* Understanding of homelessness issues and ability to plan and implement programs.
* Full driver’s license.
