Tackling Homelessness and Supporting Recovery in Kilkenny
At our organization, we are committed to addressing the complexities of homelessness and supporting individuals with disabilities or undefined life challenges in achieving their housing and recovery goals.
We understand that accessing homeless services can be challenging due to family breakdown, experiences of violence, and other vulnerabilities. Individuals with sensory and development difficulties or with a disability in general can find this an impossible space to navigate to successful outcomes.
Our pilot project aims to address these issues by working in partnership with The Collective Sensory Group, supported by the HSE Disability Services. We operate within the framework of overall objectives and policies, ensuring support for service users in accessing recovery-focused training, education, and employment opportunities.
Key Roles and Responsibilities:
1. Work collaboratively as part of a multi-disciplinary team and service.
2. Maintain a caseload of clients and offer ongoing support until cases are handed over to appropriate support services.
3. Supervise health care workers and maintain accurate records of client progress.
4. Work closely with homecare support workers to coordinate care plans.
5. Engage with service users to facilitate them in defining and attaining their own housing and recovery goals.
