About the Role

Responsibilities
* Plan, develop, and implement educational, recreational, and occupational programs within the community.
* Liaise with families, healthcare teams, and community personnel to access local resources.
* Document information, update records, and complete necessary paperwork for communication and accountability.
* Ensure interactions with service users are respectful, dignified, and equality-focused.
Requirements
* Prior experience working in the community or with individuals with disabilities.
* Ability to provide personal care and support individuals with independent living skills.
* Strong IT and reporting skills, as well as a clean driving license and access to a car for business purposes.
* Flexible schedule to meet the needs of service users.
