Job Summary:
We are seeking a dynamic Programmes Facilitator to join our team.
The successful candidate will be responsible for developing and delivering person-centred programmes to meet the needs of service users, as identified through Person Centred Planning. This will involve being a key worker to service users, facilitating the Person Centred Planning process, and maintaining required records.
In addition, the Programme Facilitator will support individuals and groups in accessing their community to meet identified needs, by facilitating activities and programmes, and performing any other reasonable duties deemed appropriate by the Programmes Supervisor/Community Services Manager.
Key Responsibilities:
* Develop and deliver person-centred programmes to meet the needs of service users.
* Be a key worker to service users, facilitating the Person Centred Planning process.
* Support individuals and groups in accessing their community to meet identified needs.
* Perform any other reasonable duties deemed appropriate by the Programmes Supervisor/Community Services Manager.
Requirements:
* Minimum QQI Level 5 in Social Care or equivalent qualification.
* Good knowledge of New Directions and community-based programmes.
* Knowledge and understanding of intellectual disabilities and mental health issues.
* 1 year minimum experience of working in a relevant setting with experience of facilitating and implementing person-centred plans.
