Project Coordinator Galway Opportunity
Rehabilitation & Nursing Home Centre
Galway
An excellent rehabilitation & nursing home centre in Galway has an opportunity for a Project Coordinator.
Key Responsibilities
* Project Coordination & Oversight
o Lead and coordinate all phases of the HSE rehabilitation bed project, from patient admissions through to discharge.
o Manage day-to-day operations to ensure smooth delivery of rehabilitation services within the nursing home.
o Ensure alignment of all activities with the agreed contractual obligations and project timelines.
o Build and maintain strong working relationships with the multidisciplinary team (MDT), including nursing, medical, physiotherapy, occupational therapy, social work, and HSE representatives.
o Act as a key liaison between internal teams, the HSE, and external partners to ensure effective communication and collaboration.
o Facilitate and foster a culture of teamwork and shared accountability across all disciplines involved in the care pathway.
* KPI Monitoring & Reporting
o Develop, track, and report on weekly KPIs to monitor performance, patient outcomes, and compliance with contract terms.
o Identify trends and variances in service delivery and take appropriate action to address any issues or inefficiencies.
o Prepare regular updates and performance reports for internal management and HSE stakeholders.
* Meetings & Communication
o Lead and participate in weekly MDT and project review meetings, ensuring all updates, concerns, and actions are documented and followed up.
o Communicate changes, updates, or issues promptly to relevant teams and stakeholders.

Job Requirements:
* Qualified Nurse with at least 3 years management experience in the last 6 years in care of the older person.
* Post registration management qualification in health or a related field.
* Proven track record in healthcare clinical management.
* Excellent leadership, organizational, and communication skills.
* Sound decision-making ability and results-driven.
* Ability to work well and promote a team environment.
* Willingness to be flexible and adapt to the needs of the company.
