Clinical Care:
Develop, review and monitor client's person-centred care plans.
Perform clinical duties as required working within your scope of practice.
Administer medications and treatments as prescribed.
Provide wound care, catheter care, palliative care, and chronic disease management.
Monitor and evaluate client progress, adjusting care plans as needed.
Respond to clinical emergencies and escalate appropriately.
Conduct risk assessments in the home and mitigate risks / escalate any issues
appropriately
Coordination & Communication:
Liaise with GPs, hospitals, allied health professionals, and family members.
Maintain accurate and timely documentation in line with clinical governance standards.
Participate in multidisciplinary case reviews and care planning meetings.
Quality & Safety:
Adhere to infection prevention and control protocols.
Ensure the care being delivered in client's homes is meeting HCCI, ISO & Dovida
Standards, local policies and the requirements of the Health Service Executive (HSE)
HIQA and Health and Safety Authority.
Report incidents, near misses, and safeguarding concerns promptly.
Education & Support:
Educate clients and families on health conditions, medications, and self-care.
Support caregivers with training and emotional support.
Participate in continuous professional development and training.
Essential: · Registered nurse (RGN, RNID, RCN) with NMBI registration.
Minimum 3 years post-registration experience, ideally in community or home care.
Full clean driving licence and access to a car.
Excellent communication and interpersonal skills.
Experience in care planning and sourcing evidence-based practice
Willingness to take on explicit clinical governance of cases with support from clinical lead in national office and General Manager disability as relevant
Business acumen to ensure client satisfaction and uphold brand reputation.
· Positive and compassionate attitude Desirable: · Experience in palliative care, chronic disease management, or gerontology.
Postgraduate qualification in community nursing or related field.
Familiarity with electronic health records and mobile documentation tools.
