About the Role
To promote and support independent living and community integration, we are seeking a skilled facilitator to join our team. The ideal candidate will have experience in designing, developing, and implementing programs and activities tailored to meet the needs and abilities of individuals with disabilities.
Key Responsibilities
* Facilitate person-centered planning processes to identify goals and supports required by individual service users
* Develop and deliver personalized programs to meet the needs of service users as identified through person-centered planning
* Collaborate with colleagues to provide comprehensive support to individuals with disabilities
Requirements
* Minimum QQI Level 5 in Health and Social Care or equivalent is desirable
* Additional qualification in supporting adults with Autism or Challenging Behaviour would be beneficial
* 1 year minimum experience of working in a relevant setting with experience in facilitating and implementing person-centered plans
