Job Title: Community Connector / Social Care Worker – Day ServiceContract Type: Full-time Permanent, Monday to Friday, 8.00am – 4.30pmPosition Location: The primary base for the position will be in Bray, Co. Wicklow. The successful candidate will be required to travel and work across the organisation delivering relevant programmes. This will include our Dun Laoghaire base and two centres in Bray.Reporting to: Vocational Training & Day Services ManagerPrimary Purpose of this Role:Day Services Programme Delivery:Key working responsibilities of assigned service membersResponsible for the implementation of person-centred plans for individuals accessing the services, including the setting, monitoring and facilitation of action plan meetings.Responsible for timetabling person-centred planning meetings and follow up progress reports,Develop behaviour support plans and progress updates as appropriate,To deliver relevant programmes/classes with an emphasis on community inclusion.Direct liaison person for service member family/ or support personsResponsible for the contribution to and adherence to weekly programme timetables.To support Service Members in having meaningful social roles in their communities with an emphasis on Supported Employment,Programme Monitoring and EvaluationContribute to the ongoing programme monitoring and evaluation as well as supporting its continuous improvement,Collect data to measure outcomes, prepare reports as requested,Adhere to quality assurance systems and procedures,Participate in committees as directed by Vocational Training and Day Services Manager / Day Services Team Leaders.Ensure all programme files are maintained to a high standard and in compliance with HSE and data protection regulatory requirementsStakeholder Engagement and CommunicationContinually build strong relationships with Service members, families, and caregivers, ensuring clear communication and satisfaction.Partner with referral sources, community organisations, and healthcare providers to enhance service offerings and continuity of care.Training and DevelopmentCarry out an identification of learner needs for assigned Service Member as part of their Person-Centred plan.To keep informed about current issues affecting people with intellectual disabilities and to integrate evidenced based knowledge into daily practiceParticipate in weekly staff and planning meetings to ensure effective communication, quality assurance of service delivery and continuous improvement of overall services programme.Health & SafetyObserve, report and take appropriate action on any matter which may be detrimental to service members' care or wellbeing,Complete incident reporting and near miss events,Complete and review risk assessments, including the development of a positive risk management plan for service members,Adhere to all HSE policies and procedures and to all relevant Health & Safety legislation.Skills, Knowledge & Abilities:Strong interpersonal and communication skills both individually and across a team setting,Strong ability to empower individuals, promoting independence and person-centred decision-making,Ability to build and maintain relationships with diverse groups,Enjoys the outdoor environment and be physically able and fit for the strenuous nature of the post.Experience & QualificationDesired: an appropriate third level qualification in a related field e.g. health and social services, disability management, allied health and social care, special education and/or specialist training etc.,Minimum: an appropriate Level 6 qualification in related field of health and social services,Provenexperience of working in disability services,Proven experience designing, facilitating and implementing Person Centred Plans in line with New Directions Policy,Proven experience in the development of behaviour support plans,Proven experience of supporting people with disabilities to access employment opportunities,Full and clean Irish driving licence.Language: Fluent EnglishBenefits of Employment:Salary: €34,081 - €49,864Benefits Include:Access to professional training and continuous development programmes.Access to Educational Support22 days' annual leave pro ratedBike to work schemeOn-site parkingApplication Process:Candidates are invited to submit their interest by 22nd Jan 2026Equal Opportunities Statement:Festina Lente is an equal opportunity employer.
