Occupational Therapist Role in Primary Care
We are seeking a motivated and skilled Occupational Therapist to join our Primary Care team. As an Occupational Therapist, you will play a vital role in supporting patients' independence and well-being.
The successful candidate will have strong clinical reasoning and problem-solving skills, with the ability to work independently and as part of a multidisciplinary team. You will be responsible for conducting assessments, developing treatment plans, and implementing interventions for service users.
* Conduct comprehensive assessments and develop tailored treatment plans to support clients' independence and well-being.
* Manage a caseload and work collaboratively with patients, families, and the wider healthcare team.
* Perform home visits to assess living environments and recommend adaptations to enhance daily functioning.
* Collaborate with multidisciplinary teams to deliver holistic care and rehabilitation services.
* Participate in clinical meetings, case conferences, and multidisciplinary collaborations.
* Maintain accurate records and documentation in line with professional standards.
Key qualifications include CORU Registration (or eligibility for registration) and a recognised Occupational Therapy qualification. Previous community experience is advantageous but not essential.
